HOW TO SCHEDULE testing, treatment and follow-up test (typically 4 days total)

Please Send:

1. Requisition/Prescription for MRI- Please obtain an prescription for"MRI diagnosis MS" from any of your physicians

2. Kidney Function Normal- Please obtain Blood BUN and Creatinine, or a note from your doctor indicating "Kidney Function Normal".

Once we receive both of the documents, you will be contacted and scheduled for testing, treatment and follow-up (typically 4 days total).

PREPARING FOR YOUR TEST

1) Please bring a check for $3,500 made out to the Applied fMRI Institute.

2) If you would like to bring a DVD, we can play it while you are having the test.

3) Sedatives and caffeine will interefere with the functional BOLD portion of the test. We ask that you refrain from taking these medications for two hours before scanning.

4) Remove any metal, such as piercing and metal in bra. You will be asked to change in to hospital scrubs.

PREPARING FOR CATHETER ANGIOGRAPHY (different location, following day)

1) Please bring your insurance card (if applicable) and credit card or check, the cost is $7,500.

2) Please bring a copy of your most recent Doctor's notes- the front desk of your physician's office will know what you are asking for.

*Please Note, our IRB approved angioplasty requires that you have your testing done at our facility. If you have already had the test done somewhere else, you will need to have it done again here. Also, you can go to your own IR.

Post-Angioplasty Follow Up Evaluation: This follow up test is required by our IRB, but is at no extra cost to you. The test takes 20 minutes and will be done the day following your treatment.
