  1. Referral Request Form-  to be completed by patient's physician (neuro, GP, acupuncturist etc.). Referrals from MDs outside the US are accepted.
  2. Patient Contact Form-    to be completed by patient or patient's representative.

PREPARING FOR YOUR CCSVI TEST

LOCATION:
The Applied fMRI Institute, 10065 Old Grove Rd. Suite 103 (back of the building), San Diego, CA 92131

Please arrive 30 minutes prior to testing

1) Please bring a check made out to the Applied fMRI Institute. (treatment billed separately)
Methods of payment:

  • VISA/Mastercard only*
  • Personal check (US only)
  • Cashiers or travelers check made out the the Applied fMRI Institute.

*Remember to notify your credit card company if you plan on making a large purchase.

2) If you are in a wheelchair and/or need assistance walking, you will need to bring a companion to help you get from your chair to the MRI bed (10 feet). We have an MRI safe wheelchair to help with the transfer, weight limit: 250lbs

3) To make you  more comfortable, you may bring a DVD or CD to watch/listen to during your MRI

4) Sedatives and caffeine will interefere with the functional BOLD portion of the test. We ask that you refrain from taking these medications for two hours prior to scanning.

5) Remove any metal, such as piercings or metal in clothing. You will be asked to change in to hospital scrubs.

6) We do not provide travel arrangements. Please visit FAQs to see nearby hotels and cab services.

7) For people who have previously been treated with a stent for CCSVI, please provide us with full documentation about said stent.

8) For patients who may experience incontinence during the test, please wear disposable briefs. Please notify the technician if you have an external collection device.


PREPARING FOR CATHETER ANGIOGRAPHY, CCSVI Liberation Therapy (separate location on the following day)

During the scheduling process, MIT Del Mar Vein Care will ask you to provide more information about medications and medical history, and the nurse will be available to answer any of your questions about the procedure itself
